Report suppressions in the files no rule is ever run over

A suppression that answers for nothing is an error, and that is what makes
on-by-default with unlimited local escapes safe. Three files fell outside it,
each one silent: a preprocessor's input, whose Haskell only exists at build
time; an unclaimed file spelled with a preprocessor's extension, which the walk
over unclaimed files did not look at; and any unclaimed file whose suppression
was written in the bare form, which the walk's hand-written marker test did not
match.

The first two are now scanned, and the marker test is the parser's own rather
than a second spelling of it, so a form one end judges and the other end misses
cannot exist. A file under no source directory at all is outside what a package
run reads and is still not covered.
