Read sqitch.plan once per group of tests that is sized from it

Sizing the timeout needs the plan, and so does the per-change check, so
reading it in both meant reading the same file twice for one group of
tests. The spec now reads it once and hands the steps to both, which
also removes the chance of the two disagreeing.

`withPlanScaledTimeout` takes the steps rather than the settings, so it
no longer reads anything, and `readSqitchPlanSteps` is exported for
callers that need the steps to pass it.
